This file is for those who are new to the process or have not turned in an application recently. It contains policies, models, definitions and other guidance, as well as the application forms. All subsequent forms on this page are included herein.

For those who are already familiar with the application process or have a copy of the Human Subjects Application Guidelines, the Outline for Application Proposal and Investigator's Assurance documents can be downloaded below. Together, these form the application for approval of research involving human subjects.

Models of Informed Consent (Required Elements and Models)
It is essential that consent forms contain all of the required elements. This file includes information about the need for informed consent in Human Subjects research, and provides examples of adult, child, and sensative populations consent forms.

Continuing Review Report (Renewal of approval)
Approval for Human Subjects research lasts for only one year. To keep the approval status current, a Continuing Review Report must be submitted each year two months prior to the expiration date of the project.

Review Not Required (Must meet requirements!)
To be used only if project involves secondary data analysis and meets all four of the criteria specified on the form. A one-page summary of the project must be included.
